Dangerous goods declaration form: ADR transport document and multimodal form

Dangerous goods sent by road need a transport document with each entry described as ADR 5.4.1.1.1 orders it; with a sea leg, shippers use the Multimodal Dangerous Goods Form. Pick each UN number and its Table A entry, enter packages and quantities, and the kit writes the documents in Word with the descriptions taken from the list itself.

Languages of the transport document

ADR 5.4.1.4.1: an official language of the country the goods leave from and, if that is not English, French or German, also one of those three.

Consignor
Consignee
The document then shows the total quantity and calculated value per transport category, worked out from your quantities.
Adds the Multimodal Dangerous Goods Form with the container/vehicle packing certificate.

Entry 1

Four digits, with or without "UN", for example UN 1993.

What is in the files

  • the ADR transport document in English, French or German: consignor, consignee and, for each entry, the UN number, proper shipping name, label numbers, packing group and tunnel code in the order of ADR 5.4.1.1.1, then the packages and the total quantity;
  • "WASTE", "EMPTY, UNCLEANED" and "ENVIRONMENTALLY HAZARDOUS" where you say so, worded as ADR gives them in that language, and the technical name after N.O.S. entries (ADR 3.1.2.8.1);
  • for Class 1, the total net explosive mass per UN number and for the consignment (ADR 5.4.1.2.1);
  • if you apply ADR 1.1.3.6, the total quantity and the calculated value per transport category, worked out from your quantities without rounding, with whether the sum stays within 1 000;
  • with a sea leg, the Multimodal Dangerous Goods Form of the UN Recommendations (also ADR 5.4.5) with the shipper's declaration and the container/vehicle packing certificate, and a read-me.

Checks the kit makes

Every entry comes from ADR 2025 Table A, as published in the BAM dangerous goods database: a UN number that is not in it, or whose carriage is prohibited, is refused. A quantity written with a thousands separator ("1,500") is refused instead of being read as 1.5 or 1500. A UN number in transport category 0 stops the 1.1.3.6 calculation, because no exemption applies to it. Anything missing prints TO COMPLETE with the reason.

What the kit does not do

It does not classify your goods or choose the UN number, entry or packing group: section 14 of your safety data sheet gives them. It does not cover tanks, bulk, limited quantities, Class 7, temperature-controlled goods or the additional entries of the IMDG Code for the sea leg (such as flash point and EmS). It does not sign: the consignor checks and signs. To look up a single number first, use the free UN number lookup.
